It’s that time of the year when employers must complete their OSHA form 300 Log of Work-Related Injuries and Illnesses and post the OSHA form 300A Summary of Work-Related Injuries and Illnesses for the 2017 calendar year.

OSHA Recordkeeping Requirements

The OSHA forms 300 and 300A must be completed even if you did not have any work-related injuries or illnesses during the 2017 calendar year.

Once you have reviewed the OSHA form 300 Log and verified that the entries are complete and accurate, you must then complete the OSHA form 300A Summary which requests the totals from the categories on the OSHA form 300 Log.

When the Log and Summary have been completed, the Summary must be posted in a visible location where your employees can view the injury and illness information. The Summary must be posted from February 1, 2018 to April 30, 2018.

The OSHA forms 300 and 300A can be obtained at OSHA’s website, www.osha.gov found under the Record keeping Link. Minnesota Contractors Premium Adjustment Program (MCPAP) Enrollment for January 1, 2019 Effective Date

The Minnesota Workers’ Compensation Insurers Association (MWCIA) recently sent all eligible contractors a reminder to enroll in the 2019 Minnesota Contractors’ Premium Adjustment Program (MCPAP). The enrollment deadline is April 1, 2018. In the event that you have not received your form, please contact our agency immediately, or visit the MWCIA website at www.mwcia.org to enroll. Please see the timetable listed below for this program:

After April 1, 2018 A late processing penalty of (.02) will apply after April 1st deadline. Penalty capped at a MCPAP factor of 1.00.

October 15, 2018
Calculation and publication of individual employers credits.

January 1, 2019
Commencement of use of MCPAP credits on new and renewal policies.

We urge you to complete the new enrollment form and mail it back to the MWCIA by April 1, 2018. Be sure to keep a copy of your form and backup data as it must be reviewed at the time your final Workers’ Compensation audit is completed.
